Oregon Workers' Compensation Injuries Are Covered for Chiropractic Care
Chiropractic care for on-the-job injuries is covered by the state of Oregon. You do not have to go through another provider or be referred by a medical doctor, lawyer or your employer. You can come to us directly and begin care. Also, if you are currently under care and your injury is not resolving, you are permitted to change providers at your own discretion without referral.

To initiate a claim, you will need to fill out a "Report of Job Injury or Illness" Form 801 available from your employer. Keep a copy and give them the original. Please bring the copy to your appointment. We have all additional forms you will need.
